Cagrilintide doesn’t get its own dedicated forum here since it’s almost always studied and discussed alongside semaglutide, but it deserves its own thread — the trial data is genuinely different from a GLP-1-only compound.

    What Is Cagrilintide?

Cagrilintide is a long-acting amylin analog — a dual amylin/calcitonin receptor agonist, developed by Novo Nordisk. That’s a different receptor system than GLP-1 entirely (see the GLP-1/GIP basics thread if you haven’t read that yet). Amylin is co-secreted with insulin by pancreatic beta cells and independently regulates satiety and gastric emptying, which is why pairing an amylin analog with a GLP-1 agonist produces an additive rather than redundant effect.

    CagriSema: The Combination

“CagriSema” refers to cagrilintide dosed together with semaglutide in a single weekly injection. This isn’t a hypothetical stack — it’s the subject of Novo Nordisk’s REDEFINE phase 3a trial program, with results published in the New England Journal of Medicine and presented at ADA 2025.

    REDEFINE 1 Results

    • Mean body weight reduction of 20.4% with CagriSema at week 68, vs. 3.0% with placebo
    • 60% of participants achieved at least 20% weight loss
    • 23% lost 30% or more
    • 88% of participants with prediabetes returned to normoglycemia
    • Significant improvements in systolic blood pressure, waist circumference, and lipid levels

These are the largest published weight-loss figures of any GLP-1-adjacent combination to date, which is why CagriSema is getting so much attention right now. Additional REDEFINE trials are underway through 2028 covering kidney disease, muscle health, and different dosing regimens.
