KeymasterRetatrutide is one of the most closely watched investigational peptides in metabolic research right now. This post pulls together what has actually been published about its mechanism of action and clinical trial results, separates completed findings from ongoing work, and flags questions that remain open.
What Retatrutide Is
Retatrutide (Eli Lilly’s LY3437943) is a single peptide engineered to activate three separate receptors: GLP-1, GIP, and glucagon. Combining agonism at all three is the core research hypothesis behind its metabolic effects — GLP-1 and GIP agonism are associated with appetite suppression and improved insulin sensitivity, while glucagon receptor agonism is theorized to increase energy expenditure, a mechanism not present in GLP-1-only or GLP-1/GIP dual agonists such as semaglutide or tirzepatide.
What Published Research Shows
A phase 2 trial published in the New England Journal of Medicine (Jastreboff et al., 2023) followed 338 adults with obesity or overweight over 48 weeks and reported dose-dependent weight reduction, with the highest studied dose (12 mg) associated with substantially greater mean weight loss than placebo.
A phase 2a sub-study published in Nature Medicine (2024) examined participants with elevated liver fat and reported large reductions in liver fat content at the higher studied doses, with paired liver biopsies used to assess markers of inflammation alongside fat content — one of the more rigorous designs used in this compound class so far.
More recently, Lilly has reported topline phase 3 results under the TRIUMPH and TRANSCEND labels. TRIUMPH-4, a 68-week trial in participants with knee osteoarthritis and obesity, reported average weight loss of roughly 26–29% across doses along with meaningful reductions in WOMAC pain scores — but it also reported a dose-dependent safety signal, dysesthesia (an abnormal skin sensation), in up to roughly one in five participants on the highest dose versus under 1% on placebo. TRANSCEND-T2D-1, a 40-week phase 3 trial in adults with type 2 diabetes, reported A1C reductions of up to 2.0 percentage points and weight loss of up to 16.8% at 40 weeks; these results were presented at the 2026 ADA Scientific Sessions and published in The Lancet.
What’s Still Unknown
Retatrutide is not approved by the FDA or any other regulatory body for any indication as of this writing. Long-term safety data beyond the current trial periods isn’t yet available, and dedicated cardiovascular outcomes data is still pending. The dysesthesia signal seen in TRIUMPH-4 is a good example of why continued monitoring across the full phase 3 program matters before drawing firm safety conclusions.
Common Misunderstandings
A few things worth flagging: trial results describe average outcomes in supervised clinical trial populations under specific dosing protocols — they are not a guide to expected outcomes for any individual outside a trial. Participation in a published trial is not the same as regulatory approval; retatrutide remains an investigational compound. And “triple agonist” describes the receptor-binding mechanism under study, not a settled conclusion about how much each receptor contributes to the outcomes measured.
