    Where semaglutide fits

    Semaglutide is a single-receptor GLP-1 agonist. It was the first of this newer generation of incretin-based therapies to demonstrate large, sustained weight loss in trials (STEP 1, NEJM 2021: 14.9% average weight loss at 68 weeks on 2.4 mg) and has since accumulated one of the broadest evidence bases in the class, including dedicated cardiovascular outcomes data.

    What semaglutide has that tirzepatide currently doesn’t

    Ozempic carries an FDA approval specifically for reducing the risk of worsening kidney disease and cardiovascular death in adults with type 2 diabetes and chronic kidney disease (the FLOW trial). Wegovy carries an FDA approval for reducing cardiovascular death, heart attack, and stroke risk in adults with cardiovascular disease and obesity/overweight (the SELECT trial, roughly a 20% relative risk reduction). Wegovy is also now available as an oral tablet — the first oral GLP-1 approved for weight management.

    What tirzepatide has that semaglutide currently doesn’t

    Tirzepatide’s added GIP-receptor activity is associated with larger average weight loss in its own trials, and Zepbound holds an FDA approval for obstructive sleep apnea that semaglutide does not have.

    The honest caveat

    None of the weight-loss percentages you’ll see cited for either compound come from a trial that put them head-to-head in the same study. They’re separate trials, separate populations — useful for understanding each compound on its own terms, not for a precise ranking between them.
